Dakar on-call pharmacy: an essential, poorly organized service in 2026

On-call pharmacies in Dakar handle around 8,000 urgent medication requests daily outside regular hours (6 PM-8 AM and Sundays/holidays). The service is mandatory in the Senegalese system: each pharmaceutical district organizes an on-call rotation. But the patient experience remains poor:

  • No clear information on which pharmacy is on call tonight
  • No way to check medication availability before traveling
  • No organized home delivery
  • Long queues, cash-only payment

Sénégalaise Plus pharmacy, in Sacré-Cœur, contacted me in March 2024 wanting to transform its on-call shifts into a structured home delivery service. It handled 28 orders on average during an on-call shift (typically 4-6 times per month). Fourteen months later, it handles 140 orders on average per shift, and became the reference pharmacy for nighttime delivery in 5 Dakar neighborhoods (Sacré-Cœur, Mermoz, Liberté, Sicap, Point E). Here is how.

H2: The dedicated on-call pharmacy + delivery site

The existing pharmacy site was a simple showcase with hours and address. Full redesign toward an order-and-delivery oriented site.

Home page with "On call tonight yes/no" status. Dynamic component displaying prominently: "🟢 On call tonight, delivery available until 5 AM" or "🔴 Not on call tonight, next shift Sunday May 25". The patient knows in 2 seconds if they can order.

Medication catalog with search engine. Database of 4,800 most-prescribed Senegalese medications, with: brand name + DCI name, dose, packaging, official price, standard indication, stock availability (real-time updated via internal tool). Patients can check before ordering if the medication is in stock.

3-step order form. Step 1: add medications to cart (with optional prescription photo for pharmacist verification). Step 2: select delivery address (5 covered neighborhoods + extended zones on request). Step 3: payment (Wave, OM, Free Money, Stripe).

Integrated pharmaceutical advice. "Ask the pharmacist" button opening a direct WhatsApp conversation with a registered pharmacist on call (2-pharmacist rotation at night). Average response time: 3 minutes.

H2: The structured WhatsApp ordering system

Out of 140 nightly orders, 65% go through the site (direct order) and 35% through WhatsApp (patients who prefer talking to someone, especially in emergencies). The WhatsApp system is calibrated to handle this volume without bottlenecks.

Single WhatsApp Business number +221 77 ... with 4 operators in night rotation. Each message is routed by keyword: "order" → order operator, "advice" → on-call pharmacist, "delivery" → courier coordinator, "emergency" → titular pharmacist (rare, 2-4 times per shift).

First-message response time: under 4 minutes guaranteed by operational SLA. Operators use a custom tool handling simultaneous orders, archiving conversations, and syncing with stock.

WhatsApp order confirmation with delivery ETA. Once validated, the patient receives: assigned courier photo + motorbike plate + estimated arrival time (computed by distance via Google Maps API).

H2: The motorbike courier network — the operational key

An on-call pharmacy without fast delivery creates no value. Sénégalaise Plus structured a network of 12 motorbike couriers delivering ONLY during shifts (4-6 nights/month on average per courier).

Courier economics. Per-delivery fee: 1,500 FCFA for close deliveries (< 5 km), 2,500 FCFA for medium (5-12 km), 4,000 FCFA for extended (12-25 km). A courier makes 8-15 deliveries per shift, or 18-50 KFCFA / night. For a courier doing 5 shifts/month, that is 100-250 KFCFA/month additional income (half a Senegalese SMIG).

Custom courier app. Each courier has a smartphone web app: receives the delivery order (with Google Maps address), confirms pickup at the pharmacy, marks "en route", marks "delivered + receipt photo", receives payment (cash or pre-collected electronic). App dev cost: 2.8 million FCFA.

Safety. Couriers wear a branded pharmacy vest + ID badge with photo. For sensitive zones or deep-night deliveries (1-5 AM), a specific protocol (paired courier + pre-call to client) limits risk.
