The verdict in three sentences
For a lab handling 250 patients a day, manual result entry costs at least two full-time staff and lets through 1 to 3% transcription errors. A custom LIS built by a regional team, at 15 to 35 million FCFA (about 27,000 to 62,000 USD), connects the analysers, sends results by SMS and patient portal and goes live in 5 months. Compared with an imported LIS billed in euros, it usually costs two to three times less over 5 years, provided analyser interfacing is properly scoped.
What an LIS does and why manual entry is expensive
A laboratory information system follows the sample from collection to validated report: patient and request registration, barcode labelling, worklists sent to analysers, automatic result capture, technical then medical validation, billing (patient, insurer, health scheme) and result delivery.
| Step | Manual entry | Connected LIS | Estimated gain |
|---|---|---|---|
| Patient registration | Paper register or Excel, 4 min | Digital record and barcode label, 2 min | 8 h a day over 250 patients |
| Result transfer | Copied from the analyser, 3 min per file | Automatic via ASTM or HL7 interface | 12.5 h a day |
| Transcription errors | 1 to 3% of reports | Close to 0% on interfaced parameters | Lower clinical risk |
| Turnaround | Collected at the desk, D+1 | SMS and portal on validation, D0 | 40 to 60% fewer desk visits |
| Insurance billing | Manual claim sheets, frequent rejections | Claims generated per insurer | Rejections halved |
| Quality traceability | Binders | Timestamped log, QC records | Ready for ISO 15189 work |
The most expensive item is copying from analysers: at 3 minutes per file, 250 files represent 12.5 hours of work a day.
Imported LIS versus local development
| Criterion | Imported LIS (European or Indian vendor) | Custom LIS built regionally |
|---|---|---|
| Upfront cost | 25 to 60 million FCFA (45,000 to 107,000 USD), licence and setup | 15 to 35 million FCFA (27,000 to 62,000 USD) |
| Analyser interfacing | 1 to 2.5 million FCFA per analyser | Included in the fixed price, 6 to 10 analysers |
| Annual subscription or maintenance | 15 to 20% of licence, paid in euros | 12 to 15% of development, local currency |
| Local billing (insurers, national schemes) | Custom work billed separately | Planned from scoping |
| SMS and mobile money payment | Rarely available | Built in |
| Support | Remote, time-zone gap | Team reachable in the region |
| Go-live | 6 to 12 months | 5 months |
Over 5 years, an imported LIS at 40 million FCFA plus 7 million annual maintenance and 8 analysers interfaced at 1.5 million each comes to about 87 million FCFA (155,000 USD). A custom LIS at 25 million plus 3.5 million annual maintenance totals 42.5 million FCFA (76,000 USD).
The 5-month plan
| Phase | Duration | Deliverable |
|---|---|---|
| Scoping | 3 weeks | Map of tests, analysers and insurers |
| Patient and file core | 5 weeks | Reception, requests, barcodes, worklists |
| Analyser interfacing | 5 weeks | ASTM or HL7 link to priority analysers |
| Validation and delivery | 3 weeks | Medical validation, SMS, patient portal |
| Billing and training | 3 weeks | Claims per insurer, 2-day training |
| Supported start | 2 weeks | On-site presence, fixes |
Also plan a UPS and a backup connection: an LIS is only worth something if it stays available during opening hours.

Kolonell builds websites that attract clients, optimized for the Sénégalese market. Free quote in 2 minutes.
Dr Kwame Mensah, director of a diagnostic lab in Accra, sees 250 patients a day, 26 days a month. Two clerks copy results full time, costing the equivalent of 560,000 FCFA a month, or 6.7 million FCFA (12,000 USD) a year. The custom LIS is quoted at 24 million FCFA (43,000 USD), with 3.6 million annual maintenance. The two clerks move to reception, allowing 3 more patients a day at an average ticket of 15,000 FCFA, about 14 million FCFA of extra revenue a year. Gross annual gain: 20.7 million FCFA; net gain after maintenance: 17.1 million. Payback in about 17 months.
FAQ
Which analysers can connect to a custom LIS?
Most recent chemistry, haematology and immunology analysers communicate via ASTM or HL7. Each interface takes 3 to 8 days of development and testing. Very old devices with no serial or network port stay on manual entry.
Are results sent by SMS confidential?
The SMS only contains a secure link and a one-time code, never the result itself. Cost is about 15 to 25 FCFA (3 to 4 US cents) per SMS. The portal logs every access.
Do we need an on-site server?
Cloud hosting is often enough, at 60,000 to 150,000 FCFA (110 to 270 USD) a month, with a local fallback mode during internet outages. Multi-site labs gain a single patient record.
Does the LIS help prepare ISO 15189 accreditation?
Yes: traceability of samples, quality controls and validations, with timestamps and operator identity. It does not replace the quality programme, but it supplies roughly half of the documentary evidence required.
Can the project be paid in instalments?
Yes, payment is usually split into 4 milestones tied to deliverables: 30% at scoping, then 25%, 25% and 20% at go-live.
