The verdict in three sentences
A private laboratory processing 450 patient files a day across 4 collection sites can no longer run on standalone analysers, printed results and insurer billing in spreadsheets. An imported LIS costs, as a 2026 order of magnitude, 15 to 40 million FCFA (about EUR 22,900 to 61,000), often without support for West African mutual health schemes, private insurers or mobile money. A custom LIS built in 4 to 6 months, connected to 12 analysers and 15 payers, pays for itself through fewer billing rejections and less front-desk time.
What a LIS must do in West Africa in 2026
Our reference case is a private lab in Dakar, Senegal, but the same constraints apply in Abidjan, Bamako or Lomé. The laboratory information system follows each file end to end: patient and prescription registration, barcode tube labelling at each site, transport to the main lab, orders sent to analysers, results retrieved, technical then biological validation, publication to patient and doctor, and billing of the patient share and third-party payers. In Senegal, many employees are covered by company health schemes called IPM, alongside private insurers. For a lab pursuing ISO 15189 accreditation, the LIS must also trace every step, quality controls and non-conformities.
| Function | Typical current situation | With a connected LIS | Measurable impact |
|---|---|---|---|
| Connecting 12 analysers | 7 connected, 5 re-keyed manually | 12 connected bidirectionally | Zero transcription errors |
| Tube labelling at 4 sites | Handwritten labels | Barcode printed at collection | 1 to 2% identification errors avoided |
| Biological validation | On paper | Screen with alert rules and patient history | 30 to 40% time saved |
| Result delivery | Paper pickup at the desk | Secure link via SMS and WhatsApp, doctor portal | 15 desk hours a day saved |
| Billing 15 health schemes and insurers | Excel claim sheets | Claims generated per payer, rejection tracking | Rejections from 8% to 2% targeted |
| Patient share payment | Cash | Wave, Orange Money, card | Secure cash handling at 4 sites |
| ISO 15189 traceability | Paper logs | Timestamped audit trail | Audit prepared in 1 day |
Health data is sensitive data under Senegal's Law 2008-12 on personal data protection, and neighbouring countries have similar laws. The LIS must therefore include a declaration to the national data protection authority (CDP in Senegal), encrypted results, role-based access and controlled hosting with daily backups.
Imported LIS or custom build: the comparison
Imported LIS products, from Europe or North Africa, are mature. Their acquisition cost sits, as a 2026 order of magnitude, between 15 and 40 million FCFA, plus per-analyser connection fees, annual maintenance often indexed in euros and support that may be remote and in another time zone. Above all, their billing logic is designed for their home country's social security, not for 15 local schemes and insurers with different coverage rules.
| Criterion | Imported LIS | Kolonell custom LIS |
|---|---|---|
| Acquisition cost | 15 to 40 million FCFA (2026 order of magnitude) | Complex application: 7,000,000 to 15,000,000+ FCFA (EUR 10,670 to 22,870+) |
| Analyser connection | Per-analyser fees, about 500,000 to 1,500,000 FCFA each | Included in the scope priced on quote |
| Maintenance | 15 to 20% of licence price per year | 75,000 to 120,000 FCFA per month (EUR 114 to 183) |
| Local health schemes and insurers | Needs adapting, often paid custom work | Designed for your 15 payers |
| Wave and Orange Money payment | Rarely available | Native |
| Time to go live | 3 to 6 months | 4 to 6 months |
| Support | Often remote | Team in Dakar |
Custom is not always the answer: a single-site lab with 80 files a day is better served by a standard product. At 450 files a day across 4 sites with 15 payers, the specific part becomes the majority and justifies a tool built around your organisation.
The project over 4 to 6 months
Month one: mapping flows across the 4 sites, inventory of the 12 analysers and their communication protocols (ASTM or HL7 depending on models), and collection of the agreements with the 15 schemes and insurers. Months two to four: development in lots, starting with registration and labelling, then connecting analysers one by one, biological validation and result publication. Month five: third-party billing and mobile payment. Final month: parallel running on a pilot site, training for 30 to 40 users, gradual switchover of the other three sites.

- Files per year: 450 × 300 = 135,000
- Average bill: 18,000 FCFA, 55% paid by third parties, i.e. 135,000 × 18,000 × 55% = 1,336,500,000 FCFA billed to schemes and insurers
- Rejections permanently lost today: 3%, i.e. 40,095,000 FCFA a year
- Target with validated claims: 1%, i.e. 13,365,000 FCFA, so a gain of 26,730,000 FCFA a year (about EUR 40,700)
Add result delivery: 40% of patients come to collect results, 180 visits a day at 5 minutes each, i.e. 15 desk hours freed daily. Even at the top of the complex application range, the LIS pays back in under a year from lower rejections alone.
FAQ
Can analysers from different brands connect to the same LIS?
Yes, each analyser is connected using its protocol, usually ASTM or HL7, sometimes through middleware. Allow on average 1 to 2 weeks of development and testing per analyser model.
Are online results compliant with data protection law?
Yes, provided the declaration to the data protection authority is filed, results are encrypted and accessed through a one-time code link sent by SMS. Access is logged and the link expires, for example after 30 days.
How many third-party payers can be managed?
There is no technical limit: each scheme or insurer has its own coverage rates, caps and claim formats. Initial setup for 15 payers takes about 3 weeks of work.
Does the LIS help with ISO 15189 accreditation?
It does not replace the quality process, but it provides the required traceability: timestamps at each step, quality controls, non-conformities. Audit preparation drops from several weeks to 1 or 2 days of compilation.
What happens if a collection site loses internet?
The site keeps registering patients and printing labels in degraded mode, then syncs when the network returns. A 4G backup connection is recommended at each of the 4 sites.
